What should you know about cellulitis purple?

Purple skin with suspected cellulitis needs prompt medical assessment, especially when it is rapidly changing, very painful, blistered, numb, cold, or accompanied by fever or feeling severely unwell. Cellulitis is a deep bacterial skin infection, and its colour can vary with skin tone and the stage of inflammation. Purple or dusky colour is not a reliable way to judge severity at home.

Cellulitis commonly causes warmth, tenderness, swelling and a change in skin colour around an entry point such as a cut, insect bite, ulcer or cracked skin. On some skin tones, inflammation may look red, pink, brown, grey or purple. A purple area can also reflect bruising, bleeding under the skin, pressure, or reduced circulation rather than cellulitis itself.

A clinician may examine the skin, check how far the change has spread, feel for warmth and tenderness, and assess your general condition. They may ask about diabetes, immune problems, recent injury, surgery, eczema, or athlete’s foot because breaks between the toes can allow bacteria to enter. Purple colour alone cannot confirm the diagnosis.

Arrange same-day medical advice for a new purple, spreading, warm or painful patch that could be cellulitis. If a clinician prescribes antibiotics, take them exactly as directed and complete the prescribed course unless the prescriber changes it. Rest the affected limb and raise it when practical to help with swelling. Keep any wound clean and covered, and avoid squeezing, piercing, massaging or applying irritating products to the area.

Do not rely on a photograph or a home boundary mark to decide that treatment is working. Tell the clinician if the colour spreads, pain increases, a blister or open area appears, or you develop new symptoms. People with type 2 diabetes, poor circulation, lymphoedema or reduced immunity should seek advice promptly for a possible skin infection.

Seek emergency care if the purple or dark area is spreading quickly, the pain is severe or out of proportion to what you can see, the skin becomes numb or cold, or blisters, blackening, drainage or a crackling sensation develops. Emergency assessment is also needed for fever with shaking, confusion, faintness, fast breathing, marked weakness, or difficulty staying awake. These signs can occur with a serious spreading infection or impaired tissue blood supply.

Do not wait for a routine appointment when the infection is near the eye, on the face, or associated with rapid swelling. A fever can accompany cellulitis, but absence of fever does not rule out a significant infection.

Ask what findings support cellulitis and whether another cause of purple skin needs consideration. Ask which changes should prompt emergency care, how to protect the affected area, whether a wound, eczema or athlete’s foot may have provided an entry point, and when the area should be reassessed. Mention every medicine you take and any history of diabetes, circulation problems or medication allergy.
