Cellulitis and diaper rash: what to know

Irritant diaper dermatitis often affects skin under the nappy where moisture and friction collect. Yeast may cause a bright red rash with small spots beyond the main area. Cellulitis tends to cause skin that is increasingly warm, swollen, tender, and red, sometimes extending beyond the original rash. A clinician may need to examine the child because an inflamed nappy area can have more than one cause.

Change nappies frequently, rinse gently with lukewarm water, pat dry, and allow short periods without a nappy when practical. Avoid scrubbing, fragranced wipes, and applying leftover antibiotics or steroid creams without advice. A protective emollient can reduce moisture and friction; antifungal treatment is used only when yeast is suspected or confirmed.

Arrange prompt medical care if redness is spreading, the skin is hot or painful, swelling is increasing, blisters or pus appear, or the child seems unwell. Seek urgent help for marked sleepiness, breathing difficulty, poor feeding, or rapidly worsening skin changes.
