Cellulitis in both legs: what to know

Cellulitis commonly presents as a localized, warm, tender, expanding area of skin infection. When both legs become red or swollen, a clinician should also consider fluid-related swelling, vein problems, contact irritation, eczema, or another inflammatory condition. This does not rule cellulitis out, especially if one area is distinctly hotter, more painful, or expanding. Cracked skin from Athlete's Foot, scratches, ulcers, or surgery can provide an entry point for bacteria. Type 2 Diabetes may affect healing and sensation, so changes can be harder to judge.

Contact a healthcare professional promptly, particularly if the change is new or worsening. Keep both legs clean, avoid scratching, and cover any open area with a clean dressing. Elevate the legs when resting if that feels comfortable, but do not massage an area that is hot and painful. Avoid tight clothing and unapproved creams. Note when the redness began, whether it is spreading, and whether one leg feels different from the other. Bring a list of medicines and mention diabetes, immune problems, or previous infections.

Get urgent help for rapidly spreading redness, severe pain, marked weakness, confusion, faintness, breathing difficulty, or skin that becomes purple, grey, or black. Fever or shaking chills with painful, hot skin can indicate a more serious infection. Emergency assessment is also appropriate if swelling appears suddenly with chest pain or breathlessness. Do not use the fact that both legs are involved to delay review; the pattern needs a physical examination.

Ask what findings support cellulitis and what findings suggest a circulation or inflammatory problem instead. Ask whether either leg has an entry wound, how to monitor the borders, and which symptoms require emergency care. If treatment is prescribed, ask when improvement should begin and what to do if the redness continues to spread.
