What should you know about cccDNA in hepatitis B?

Hepatitis B uses liver cells to reproduce. During that process, viral genetic material can form a stable template called covalently closed circular DNA, or cccDNA, within the cell nucleus. Current treatment can suppress viral reproduction and protect the liver, but a stable template may remain. This helps explain why hepatitis B can need long-term monitoring and why treatment decisions cannot be based on one result alone. Blood tests, liver health, symptoms, and medical history are considered together. cccDNA is not the same as a routine measure of how much virus is in the blood.

Ask a liver specialist or clinician experienced with hepatitis B to explain which tests are being ordered and what each result means. Follow the monitoring plan, take prescribed antiviral medicine consistently, and do not stop it without medical advice. Avoid sharing razors or toothbrushes, and discuss household or sexual contacts and vaccination where relevant. Limit alcohol and check with a clinician before starting supplements or medicines that may affect the liver. A liver function test may be part of ongoing assessment. Fatigue or dark urine should be reported, especially if they are new or worsening.

Arrange prompt medical review for yellowing of the eyes or skin, worsening dark urine, marked abdominal swelling, vomiting, confusion, unusual bleeding, or rapidly increasing weakness. Seek urgent care for severe confusion, collapse, severe abdominal pain, or difficulty staying awake. People with hepatitis B may have few symptoms, so scheduled monitoring remains useful even when you feel well.

Ask whether your hepatitis B is active, which blood and liver assessments you need, whether antiviral treatment is indicated, how often monitoring should occur, and whether family or household contacts need testing or hepatitis B vaccination.
