CCCA and Scarring Alopecia: What to Know

Scarring alopecia means the follicle has been injured and replaced by scar tissue, so the opening may disappear and hair production can stop there. In CCCA, the process commonly begins centrally and extends outward. Burning, tenderness, itching, or small scalp bumps may accompany the visible thinning, although symptoms vary.

A bald patch has many possible explanations and does not identify its cause by appearance alone. A dermatologist may inspect follicle openings, review the time course, and perform a biopsy at the edge of an active area when needed.

Arrange assessment when crown thinning is persistent or expanding. Use low-tension styles, limit heat and chemical processing, and stop any practice that causes pain or scalp sensitivity. Gentle care cannot reverse established scarring, but it can reduce additional mechanical irritation.

If inflammation is active, a clinician may prescribe topical or oral treatment, or inject medicine into selected scalp areas. Follow-up helps determine whether the condition is settling. Hair restoration options are discussed only when the disease has been quiet for a sustained period.

Contact a clinician promptly for rapid enlargement, new pustules, drainage, crusting, strong pain, or pronounced redness. Sudden loss in several separate patches should also be assessed, because a different or additional condition may be involved.

Ask whether the follicles show active inflammation or established scarring, whether biopsy is needed, and how you will judge stability. Discuss safe styling, treatment reactions, and whether another cause of hair loss should be checked.
