CBD and glioma: what patients should know

Cannabidiol, usually shortened to CBD, is a cannabis-derived substance that does not produce the same intoxicating effect as THC. Some people consider it for symptoms such as discomfort, poor sleep or nausea, but symptom use is different from treating the tumour itself.

Glioma treatment is planned from the tumour type, grade, molecular features, location and the person’s overall health. Surgery, radiotherapy and medicines may be considered in different combinations. CBD should not delay imaging, tissue diagnosis, neurological assessment or a recommended treatment.

Products labelled CBD can vary in strength and may contain other cannabis compounds. CBD can also change how the body processes prescription medicines, including anti-seizure drugs, sedatives and some cancer medicines. This may alter side effects or medicine levels even when CBD was bought without a prescription.

Give the oncology team the product name, ingredient list, concentration, dose and how often you plan to use it. Include oils, gummies, capsules and inhaled products, as well as every prescribed medicine and supplement. The team can check for interactions and explain whether CBD could complicate seizure control, alertness, liver monitoring or treatment decisions.

Do not stop anti-seizure medicine, steroids, chemotherapy or another prescribed treatment in order to try CBD. If you already use CBD, record when you take it and any change in drowsiness, dizziness, diarrhoea, appetite, mood or seizures.

Seek urgent medical help for a first seizure, a seizure lasting longer than usual, repeated seizures without full recovery, new weakness, trouble speaking, severe or rapidly worsening headache, fainting, marked confusion or unusual difficulty waking. These symptoms need assessment whether or not CBD has been used.

Contact the treating team promptly after starting CBD if pronounced sleepiness, persistent vomiting or diarrhoea, yellowing of the eyes or skin, a rash, worsening balance or a change in seizure pattern develops.

Ask which symptom you are trying to manage, whether a better-tested option is available, which of your medicines could interact with CBD, and what side effects should trigger a call. Also ask whether the product could affect planned blood tests, seizure advice, driving or the timing of cancer treatment.
