Causes of Spotting Mid-Cycle

Spotting mid-cycle can result from ovulation, contraception, infection, pregnancy-related changes, or conditions affecting the cervix or womb. A small amount of bleeding does not identify the cause by itself, so the timing, pattern, other symptoms, medicines, and possibility of pregnancy all matter.

Hormone changes around ovulation can cause brief, light spotting. Starting, stopping, missing, or changing hormonal contraception may also disturb the usual bleeding pattern. A contraceptive device can contribute, particularly soon after it is placed.

Infections such as chlamydia may irritate the cervix and cause bleeding, especially after sex, with unusual discharge, pelvic pain, or pain when passing urine. Small growths such as endometrial polyps, or uterine fibroids, can also make bleeding irregular. Cervical dysplasia and cervical cancer can present with bleeding between periods; womb cancer can present with abnormal uterine bleeding. These conditions are not the only explanations, and spotting alone cannot diagnose them.

If pregnancy is possible, spotting may be pregnancy-related, but bleeding with one-sided pain, faintness, or shoulder-tip pain needs urgent assessment because an ectopic pregnancy can be serious.

Record when the spotting starts, how long it lasts, whether it follows sex, and any pain or discharge. Note recent contraception changes, medicines that affect bleeding, and the date of your last normal period. Take a pregnancy test if pregnancy is possible.

Arrange a medical review if the bleeding recurs, continues, happens after sex, or is accompanied by pelvic pain or unusual discharge. Evaluation may include a pregnancy test, pelvic examination, infection testing, cervical screening when due, blood tests, or an ultrasound, depending on your history.

Seek urgent medical help for heavy or rapidly increasing bleeding, severe or one-sided pelvic pain, fainting, marked dizziness, fever, or bleeding during pregnancy. These symptoms can signal a problem that needs prompt examination. If you have gone through menopause, any vaginal bleeding should be assessed by a clinician, even when it is light.
