Causes of hand swelling and pain

Hand swelling with pain can follow an injury, joint inflammation, infection, fluid buildup, or pressure on nearby tissues. The pattern offers useful clues: a sudden problem after a fall or twist differs from gradual stiffness affecting several finger joints. The cause cannot be confirmed from swelling alone.

Pain focused over the wrist or one finger after trauma may reflect a sprain, tendon injury, bruise, or fracture. A distal radius fracture often presents with wrist pain and swelling after a fall. Warm, tender swelling with spreading redness may accompany infection. Rheumatoid arthritis commonly presents with painful, swollen joints and morning stiffness, often involving both hands. Puffiness can also occur when fluid collects after prolonged immobility or because a ring or dressing is too tight.

Remove rings promptly before swelling increases. Rest the hand from gripping and lifting, keep it raised above heart level when practical, and use a wrapped cool pack after a recent minor injury. Avoid forceful massage, tight bandages, or trying to straighten a deformed finger or wrist. Arrange an assessment if movement remains limited, pain is worsening, or symptoms have no clear minor cause.

Seek urgent care after a significant injury if the hand looks misshapen, a wound exposes deeper tissue, or you cannot move the fingers. Prompt assessment is also needed for fingers that become blue, pale, cold, numb, or increasingly tight. Fever, rapidly spreading redness, severe warmth, pus, or red streaks can signal infection. Sudden swelling of the face or lips, breathing difficulty, or widespread hives requires emergency help.

Explain when the swelling began, whether an injury occurred, which joints hurt, and whether stiffness is worse after waking. Ask whether an examination or imaging is needed, what activity is safe, and whether a splint would help. Mention fever, skin breaks, new medicines, numbness, other swollen joints, and any difficulty removing jewellery.
