Cast treatment for a thumb fracture

This cast usually wraps the wrist and thumb while leaving the fingertips visible. Its exact shape depends on the broken bone and the position that must be protected. An initial splint may allow room for swelling before a full cast is fitted.

The thumb is central to pinch and grip, and some fractures extend into a joint or affect stability at its base. Immobilisation aims to prevent twisting and bending while preserving a useful alignment. Follow-up images can show whether the fragments remain in position inside the cast.

The cast should feel supportive without causing focal pressure. Early swelling can make it tighter; later reduction in swelling can create looseness and allow unwanted movement.

Keep the cast clean and dry, and do not alter its edges or insert anything underneath. Elevate the hand as directed and regularly check the exposed fingertips for colour, warmth and sensation. Move the free fingers gently if permitted, but do not test the injured thumb.

Avoid gripping, lifting, driving and sport until the clinician sets appropriate limits. Attend scheduled reviews even if pain improves, as the thumb can feel better before the fracture is ready for load.

Seek urgent assessment if a fingertip becomes blue, pale or cold, numbness or tingling appears, swelling rapidly increases, or pain continues to worsen after elevation and advised pain relief. New inability to move an uncovered finger also needs attention.

Report a split, wet, soft, loose or foul-smelling cast promptly. Emergency assessment is appropriate for an open wound, uncontrolled bleeding, visible bone or another injury that changes the thumb's shape.

Ask which thumb bone is fractured, whether a joint or ligament is involved, and how stable the position appears. Find out when the cast and alignment will be checked, and whether a removable splint may follow.

Before returning to daily tasks, clarify when pinching, gripping, typing, driving and sport are allowed. Ask whether hand therapy is needed to rebuild movement and strength without overloading the healing area.
