Can Sever’s disease require a cast?
A cast is sometimes used for Sever’s disease when heel pain is severe or walking remains difficult despite simpler measures. It temporarily limits ankle and heel movement, reducing repeated pulling and impact around the irritated growth area. Casting should be recommended and fitted by a qualified clinician after examining the child.
What casting means for recovery
Sever’s disease, also called calcaneal apophysitis, is irritation where the Achilles tendon and heel bone meet near a growing area. A cast does not correct a deformity or make growth finish sooner. Its purpose is to give the painful heel a protected rest when ordinary footwear, heel support and reduced activity have not provided enough comfort. The clinician decides whether the child may bear weight and how long immobilisation is appropriate.
Caring for your child during casting
Follow the cast team’s instructions about walking, crutches and bathing. Keep the cast dry, do not place objects inside it to scratch, and check the visible toes for normal colour, warmth and movement. Encourage only the activities the clinician permits. After removal, a gradual return to comfortable movement may include supportive shoes, stretching, custom orthotics or paediatric physiotherapy, depending on the examination and the child’s activities.
When to seek care
Contact the treating clinic promptly if the cast becomes cracked, wet, loose or painfully tight, or if pain is worsening rather than settling. Seek urgent assessment for toes that become blue, very pale, cold, increasingly swollen, numb or difficult to move. Fever, a bad smell, drainage, burning pain or skin soreness at the cast edge also needs medical review. New heel pain after a fall or an inability to bear weight should be assessed rather than assumed to be Sever’s disease.
