When is a cast used for a broken kneecap?

Non-surgical care is more likely when the broken pieces remain suitably aligned and the knee's extensor mechanism still works. A displaced fracture, open wound, loose joint fragment, or inability to actively straighten the knee may lead to discussion of fixation surgery.

Why knee bending is restricted

The patella sits within the quadriceps tendon and transmits force as the knee straightens. Bending the knee pulls across the healing fracture, so the support is commonly locked in extension at first.

Weight-bearing and knee movement are separate decisions. You may be allowed to stand with the knee held straight while bending remains restricted, or you may need crutches and tighter loading limits. Follow the instructions written for your particular fracture.

Safe mobility and cast care

Use crutches or a walking aid as demonstrated and keep the knee brace locked at the prescribed setting. Do not change the hinges yourself. If you have a cast, keep it dry and never place objects inside. Elevate the leg when resting and move the ankle and toes if permitted.

Avoid stairs without a safe technique, low chairs that force the knee to bend, and driving while the rigid support prevents reliable pedal control. Attend review appointments so alignment and healing can be checked before knee flexion is gradually introduced.

When to get urgent help

Seek urgent care if the foot becomes cold, pale, blue, numb, very swollen, or difficult to move, or if severe pain worsens despite elevation and prescribed medicine. A cast or brace that creates intense pressure or skin injury also needs rapid assessment.

New calf pain or swelling should be assessed promptly. Sudden breathlessness, chest pain, coughing blood, or collapse requires emergency help. Contact the treating team for fever, wound drainage, a damaged cast, a slipping brace, or a sudden loss of knee control.

Planning knee rehabilitation

Ask whether you may bear weight, how the brace should be set, and when knee bending can begin. Clarify bathing and sleeping arrangements, blood-clot precautions, work and driving restrictions, and which exercises will maintain thigh strength without pulling the fracture apart.

Can I walk while my patella fracture is in a cast?

Some stable fractures permit protected weight-bearing with the knee straight, while others do not. Use the walking aid and loading limit prescribed for you rather than testing the leg independently.

Why might a hinged brace replace a cast?

A hinged brace can keep the knee straight initially and later allow controlled increases in bending. It also permits fitting adjustments as swelling changes, but it must remain at the prescribed setting.

Why does thigh strength decrease during treatment?

The quadriceps works less while the knee is held straight and activity is reduced. Approved static muscle exercises and later rehabilitation help rebuild control once they are safe for the fracture.
