The phrase usually refers to observing the hand's resting finger pattern and the path each fingertip follows during a gentle fist. It is an assessment concept within hand therapy, not a single standard exercise or device.
How therapists use the finger cascade
In a relaxed hand, the fingers normally sit in a curved sequence. As they bend, the fingertips should travel in coordinated arcs without crossing or rotating unexpectedly. A changed cascade may reflect swelling, stiffness, tendon imbalance, joint restriction, scarring or the position of a healing fracture.
The therapist compares active and assisted movement, checks sensation and swelling, and watches a real task such as holding a cup or fastening clothing. Findings guide treatment after problems such as a distal radius fracture, Dupuytren's contracture or surgery. They do not replace the treating surgeon's movement precautions.
What to do during rehabilitation
Follow the specific limits given for healing tissue. Complete prescribed movements slowly enough to see whether a finger rotates, catches or lags, and record which activities create swelling or loss of control. More force is not necessarily more useful.
A programme may include swelling control, scar care, splinting, tendon-gliding patterns, joint movement and graded practice of daily tasks. The therapist adjusts the dose according to tissue healing and the hand's response rather than using the same routine for every diagnosis.
When the hand needs prompt review
Contact the treating team promptly if a splint causes pressure injury, fingers become cold or change colour, swelling rapidly worsens, or new numbness or loss of movement appears. A wound that opens, drains, becomes increasingly red or is accompanied by fever also needs clinical assessment.
After a new injury, urgent care is appropriate when a finger looks rotated or deformed, a tendon may have been cut, or you cannot actively bend or straighten a digit. Stop an exercise that produces sharp pain or a sudden change in function.
