The short answer
A tear may develop gradually in weakened tendon tissue or occur suddenly when an already vulnerable tendon is overloaded. A direct injury can also tear an otherwise functional tendon.
Irritation is different from tearing
Tendinitis describes a painful tendon disorder, often related to irritation or changing tendon quality. A rotator cuff tear means tendon fibres have separated partly or completely. Both can cause pain during overhead use and discomfort at night, so symptoms alone may not reveal which is present.
New weakness is more concerning for a meaningful tear, particularly when the arm drops unexpectedly or lifting ability changes after an injury. Some tears cause little pain, while severe pain can occur without a tear. Examination compares motion and strength; ultrasound or MRI may be used when the result would alter treatment.
Manage the load and rebuild capacity
Temporarily reduce repeated lifting or reaching that predictably flares symptoms, but keep the shoulder moving within a tolerable range. Rehabilitation can gradually strengthen the rotator cuff and shoulder-blade muscles, improve movement control and prepare the tendon for work or sport demands.
Arrange an assessment if weakness persists or pain keeps returning despite activity changes. Treatment decisions consider whether a tear is present, its pattern, your function and your goals. A scan is not automatically necessary for every episode of shoulder pain.
Signs that need medical care
Seek prompt care if a fall, sudden pull or heavy lift is followed by immediate weakness or inability to raise the arm. Get urgent help if the shoulder is visibly deformed, the hand becomes cold or pale, numbness is extensive, or the joint is hot and red with fever. Gradually worsening weakness, continuing night pain or loss of function warrants a routine clinical appointment.
What to ask about
Ask whether your strength loss reflects pain inhibition or a tendon defect, whether imaging would change the plan, and how to modify specific work or gym tasks. If a tear is found, discuss whether rehabilitation is a reasonable starting point and which changes would justify referral to a shoulder surgeon.
