Can a Herniated Disc Cause Cauda Equina Syndrome?

The cauda equina is a bundle of nerves at the lower end of the spinal canal. Compression can disrupt bladder, bowel, sexual, sensory, and leg function, so suspected cauda equina syndrome requires emergency assessment.

A lumbar disc has a softer centre surrounded by a tougher outer layer. If disc material pushes into the spinal canal, it may crowd the nerve roots below the spinal cord. A large central herniation is especially concerning when it compresses several of these roots together.

Cauda equina syndrome can present with new difficulty starting urination, inability to empty the bladder, loss of bladder or bowel control, reduced feeling around the genitals, buttocks or inner thighs, altered sexual function, or severe or worsening symptoms in both legs. Back pain or sciatica may also be present, but pain alone does not establish the diagnosis.

Check for new changes in urination, bowel control, saddle-area sensation, sexual function, leg strength, and walking. If any emergency warning sign is present with back or leg symptoms, go to the nearest emergency department now rather than waiting for a routine appointment. Do not drive yourself if weakness or loss of sensation makes driving unsafe.

Emergency clinicians may examine leg strength, reflexes and sensation, ask detailed questions about bladder and bowel function, and arrange urgent spinal imaging when compression is suspected. Treatment depends on the findings; confirmed nerve compression may require urgent review by a spinal surgical team.

Seek emergency care immediately for new numbness between the legs or around the genitals or anus; new trouble starting or controlling urination; loss of bowel control; new sexual-function changes; or severe, worsening weakness or numbness affecting both legs. These symptoms need prompt assessment even if they are mild, intermittent, or appeared after a previous diagnosis of a herniated disc.

Ask which changes in bladder sensation or control should trigger an emergency visit, whether your examination shows nerve weakness or sensory loss, and whether imaging is needed. If a herniated disc is already known, ask where it sits, which nerves it contacts, and how to respond if your symptoms change.
