Can this progression happen?
When this occurs, it is called post-essential thrombocythaemia myelofibrosis. The bone marrow develops scarring and may become less able to make healthy blood cells normally.
What this change means
Essential thrombocythaemia begins as a disorder in which the bone marrow makes too many platelets. If it progresses, marrow scarring can alter the production of red cells, white cells and platelets. The spleen may enlarge as it helps with blood-cell production.
Possible clues include increasing tiredness, breathlessness, night sweats, unintended weight loss, fullness or discomfort under the left ribs, and changing blood counts. These features can have other explanations, so clinicians assess the pattern rather than relying on one symptom.
What to do if you have essential thrombocythaemia
Keep scheduled haematology appointments and blood tests, even when you feel well. Tell your team about new symptoms or a clear change in stamina, appetite, weight, bruising or abdominal comfort. Your clinician may compare blood counts over time, examine your spleen and arrange further blood or bone-marrow testing if the findings suggest progression.
Continue prescribed medicines unless your treating team advises a change. A new symptom does not by itself confirm myelofibrosis, and testing is needed to distinguish progression from medicine effects, infection, bleeding or another cause.

Contact your haematology team promptly for persistent fever, drenching night sweats, unexplained weight loss, worsening fatigue, increasing fullness beneath the left ribs, unusual bruising or bleeding, or recurrent infections.
Seek urgent care for severe chest pain, sudden breathlessness, coughing up blood, fainting, new weakness on one side, trouble speaking, or heavy bleeding that does not stop. These symptoms can signal an urgent clotting or bleeding problem and should not wait for a routine appointment.
