Can atopic dermatitis cause periorbital hyperpigmentation?

Periorbital hyperpigmentation is darker colour around the eyelids or eye sockets. Atopic dermatitis may inflame this delicate skin, and scratching or rubbing can stimulate pigment production and leave colour behind after redness settles. Dryness, thickened skin, and shadowing from eyelid swelling can make the area look darker. The colour may not fade at the same pace as the itch, so improvement in the rash does not always produce an immediate cosmetic change.

Follow the treatment plan for the underlying dermatitis and avoid rubbing, even when the itch is distracting. Use a bland, fragrance-free moisturiser recommended for eyelid skin, applied gently with clean hands. Avoid new cosmetics, fragranced products, and harsh cleansers near the eyes while the skin is active. Do not apply a bleaching product, exfoliating acid, or steroid near the eyelids unless a clinician has specifically advised it; this area can react easily.

Arrange a dermatology review if the pigmentation is new, one-sided, rapidly changing, or continues despite control of the rash. Seek prompt care for eye pain, marked swelling, light sensitivity, vision changes, pus, or crusting, because these features may signal an eye problem or infection rather than pigment alone.

Ask whether the colour is pigment, residual inflammation, or shadowing; which eyelid products are suitable; and when a pigment treatment could be considered safely after the dermatitis is controlled.
