Calf pain after knee replacement: what to know

Short answer
Calf pain after knee replacement may come from postoperative swelling, altered walking, or muscle strain, but a blood clot must be considered.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Key concern
- A deep vein clot can resemble ordinary postoperative calf soreness.
- Avoid
- Deep calf massage until concerning swelling and pain have been assessed.
- Emergency signs
- Breathlessness, chest pain, coughing blood, or fainting.
The short answer
New or worsening pain deserves prompt contact with your surgical team, especially when the calf is swollen, warm, red, or tender on one side.
What the discomfort may mean
Fluid and bruising can move down the leg after the operation and make the calf feel tight. The calf muscles may also work differently while you use walking aids or protect the new joint. These explanations can resemble symptoms of deep vein thrombosis, in which a clot forms in a deep vein, so symptoms alone cannot reliably separate them.
Pain related to activity may point toward muscular overload, while marked one-sided swelling or warmth raises more concern. Your clinician may examine both legs and arrange imaging when a clot needs to be excluded.
Safe steps to take now
Follow the movement, elevation, medication, and compression instructions provided by your replacement team. Continue only the prescribed exercises that feel tolerable, and note when the pain began, whether swelling is increasing, and whether the calf looks different from the other side. Contact the surgical team promptly about unexplained new calf pain.
Do not deeply massage a painful or swollen calf and do not add strenuous stretching until a clinician has assessed possible clot symptoms. Avoid changing prescribed blood-thinning medicine without the prescriber's direction.
When urgent care is needed
Seek emergency help for sudden shortness of breath, chest pain, coughing blood, fainting, or a rapid unexplained heartbeat. These can occur if a clot travels to the lungs. Obtain urgent medical assessment for rapidly increasing one-sided calf swelling, warmth, redness, severe tenderness, a cold or pale foot, new numbness, fever, wound drainage, or pain that is escalating rather than settling.
Questions for your surgical team
Ask whether the pattern fits expected postoperative swelling, whether an examination or ultrasound is needed, which exercises should continue, and how to use prescribed compression or medication. Also clarify whom to contact outside clinic hours if the symptoms change.

Questions people ask
Is calf tightness expected after knee replacement?
Tightness can accompany swelling, bruising, and altered walking after replacement. Because a clot may cause similar symptoms, report new, worsening, or one-sided tightness to the surgical team.
Can I massage my calf after the operation?
Avoid deep massage when the calf is newly painful, swollen, warm, or red until a clinician has ruled out a clot. Follow the rehabilitation team's guidance for any hands-on treatment.
Should I stop my knee exercises?
Continue only the exercises your team prescribed and that remain comfortable. Pause an activity that sharply worsens calf pain and contact the team for individual guidance.
