A bruised or heavy feeling around the treated spot may reflect the muscle's response to the needle. It should trend toward improvement rather than become progressively more intense.
Why the calf may hurt afterward
Dry needling enters a sensitive band of muscle, so brief local irritation, a small bruise, or a cramp-like ache can follow. Pain focused near the needle sites and triggered by using or pressing the muscle is more consistent with a local treatment response.
Pain that feels different from the original problem deserves attention. Burning, tingling, numbness, new weakness, spreading redness, increasing warmth, or swelling of the whole lower leg should not be assumed to be routine soreness.

When the pain needs assessment
Contact the treating clinician if soreness is not easing, limits walking, or is accompanied by a growing bruise, persistent numbness, or weakness. Seek urgent medical care for a hot, red, markedly swollen calf, fever with worsening skin changes, or severe pain that is escalating.
Call emergency services for sudden shortness of breath, chest pain, fainting, or coughing blood. These symptoms are not an expected effect of dry needling and require immediate evaluation.
