A clinician can check bone tenderness, calf strength, ankle movement, footwear, and gait. Imaging may be considered when examination suggests a bone injury. Resume running through a gradual loading plan only after ordinary walking and the recommended strength tasks are comfortable.

Seek prompt assessment if you cannot bear weight, have marked swelling, notice a very specific area of bone tenderness, or develop pain at rest or overnight. Sudden loss of push-off strength, a snap near the heel, or a visible change in the tendon region also warrants urgent evaluation.

Emergency care is appropriate for calf pain accompanied by sudden one-sided swelling, warmth, redness, chest pain, shortness of breath, faintness, or coughing blood. New numbness, a cold or pale foot, or severe escalating pain also requires immediate help.
