What should you know about calcitonin immunohistochemistry?

Short answer
Calcitonin immunohistochemistry detects calcitonin protein in tissue and helps a pathologist classify an abnormal growth.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Sample
- Prepared tissue from a biopsy or surgery
- Target
- Calcitonin protein within cells
- Purpose
- Support classification of a tumour alongside other findings
Direct answer
A laboratory applies an antibody to a prepared tissue section. Visible staining shows where the protein is present. The pathologist assesses the staining pattern together with cell appearance, tissue structure, and other markers before reaching a conclusion.
How the stain is interpreted
Calcitonin is normally produced by specialised cells in the thyroid. Staining in tumour cells can support a diagnosis arising from that cell lineage, including medullary thyroid carcinoma, when the microscopic features and companion tests agree.
A stain is not interpreted as a simple yes-or-no cancer test. Pathologists consider which cells stain, where the signal appears inside them, how extensive or intense the pattern is, and whether internal controls worked. Unexpected, weak, or absent staining may require comparison with other markers or additional tissue.
Next steps after the result
Read the calcitonin stain within the final pathology diagnosis, not in isolation. Confirm whether the report calls the result supportive, inconclusive, or discordant with the suspected diagnosis.
Your clinician may correlate tissue staining with the location of the growth, imaging, blood calcitonin measurements, and other pathology markers. Further staining, molecular testing, or another biopsy may be considered if the tissue is limited or the results do not form a consistent picture. Treatment decisions depend on the established tumour type and extent rather than this stain alone.
When medical attention is needed
The staining result does not itself create an urgent physical problem. Seek prompt assessment for new trouble breathing, rapidly increasing neck swelling, difficulty swallowing saliva, coughing up blood, fainting, or severe weakness, regardless of whether the pathology report is complete.
Following a biopsy, contact the procedure team for worsening redness, warmth, drainage, fever, or persistent bleeding at the sampling site. Emergency services are appropriate for breathing difficulty or heavy uncontrolled bleeding.
Questions to clarify the finding
Consider asking: Which cells showed calcitonin staining? Did the controls behave as expected? Which other stains support or challenge the diagnosis? Should my blood calcitonin be checked or compared with an earlier result? Is the available tissue sufficient to guide the next decision?
Questions people ask
Does calcitonin staining alone confirm medullary thyroid carcinoma?
It can strongly support that diagnosis in the right setting, but the pathologist also evaluates tissue appearance, other markers, and relevant clinical findings.
Is tissue calcitonin staining the same as a blood calcitonin test?
No. Immunohistochemistry shows protein within a tissue section, while a blood test measures calcitonin circulating in the bloodstream.
