Cafe au lait spots and vitiligo: what to know

A cafe au lait spot is usually evenly coloured, smooth, and clearly outlined. It may be present from childhood and can occur on many body areas. Vitiligo appears as pale or milky-white patches, often with sharper contrast at the border; hair within a patch can also lose colour. A photograph can suggest a pattern, but it cannot confirm the cause. A clinician may examine the skin under specialised light and check whether pigment loss is spreading.

Take clear, dated photographs in consistent lighting and note whether a mark was present early in life, changed, or appeared after another skin problem. Use sunscreen on exposed areas because contrast can become more noticeable after tanning. Do not apply bleaching products or steroid creams without advice. If appearance is your concern, ask a dermatologist whether observation, camouflage, or pigment-targeting treatment is appropriate; picosecond laser treatment may be discussed for selected brown spots.

Arrange a dermatology review if several new cafe au lait spots appear over a short period, marks change quickly, or pale patches expand. Seek prompt care for a spot that bleeds, crusts, becomes persistently painful, or develops a markedly irregular colour or edge. New widespread pigment loss, eye symptoms, or scalp and hair changes also merit an appointment so other causes can be considered.
