Symptoms may begin with pain or altered sensation and can progress to clumsy hands, difficulty walking, or loss of strength. The pattern varies with the location, severity, and speed of pressure on the cord. New neurological symptoms in someone with cancer need prompt medical attention.

The C4-C5 level lies in the neck, where spinal cord pathways carry signals between the brain and much of the body. Pressure here may produce neck pain, pain spreading toward a shoulder or arm, tingling or numbness, weak arms or legs, stiff legs, reduced hand coordination, unsteady walking, or frequent falls. Severe compression can affect bladder or bowel control and, when high cervical function is compromised, breathing.

Cancer-related compression can result from a tumour in or near the spine, collapse of a weakened vertebra, or tissue extending into the spinal canal. Multiple myeloma can present with vertebral damage that leads to spinal cord compression. Symptoms cannot locate the exact level or identify the cause by themselves; examination and urgent spinal imaging are used to do that.

If you have cancer, a previous cancer diagnosis, or a spinal tumour and develop new neck or back pain with weakness, numbness, walking difficulty, or bladder or bowel change, contact your oncology emergency number or emergency department immediately. Explain the cancer history and say that spinal cord compression is a concern. Do not wait for a routine clinic visit.

Keep movement gentle while arranging help, particularly if pain is severe or weakness is increasing. A clinical examination and MRI commonly guide treatment. Management may include medicine to reduce swelling, radiotherapy, surgery, stabilisation, or a combination chosen for the cause, spinal stability, neurological findings, and overall health.

Call emergency services now for new or rapidly worsening limb weakness, inability to stand or walk, loss of bladder or bowel control, numbness around the buttocks or genitals, or breathing difficulty. Sudden severe neck pain after a minor movement or fall also warrants emergency evaluation when bone may be weakened. These red flags are urgent because delay can allow neurological damage to progress.

Ask which spinal level is affected, what is causing the pressure, whether the spine is stable, and how quickly treatment must begin. Clarify the aims of steroids, radiotherapy, or surgery; what movement is safe; and whom to contact if strength, sensation, walking, bladder function, bowel function, or breathing changes.
