C1 Laminectomy: Procedure and Recovery

C1, also called the atlas, supports the skull and forms a ring around the spinal cord. Surgery may be considered when narrowing or another structural problem compresses neural tissue at this level. The exact operation depends on the cause and on whether the area is stable.

Removing the posterior arch can relieve pressure, but decompression does not automatically correct every cause of compression. The surgeon reviews scans to identify bone, ligament, alignment, or soft-tissue factors and to see how the skull, C1, and C2 move in relation to each other.

If instability is present or expected, fixation and fusion may be discussed in addition to laminectomy. Fusion can protect the spinal cord by limiting harmful movement, though it also reduces motion across the fused region. A decompression without fusion may be suitable when stability can be preserved.

Before surgery, provide a complete medicine list and mention blood-thinning medicines, allergies, smoking or nicotine use, previous neck operations, swallowing difficulty, and problems with anaesthesia. Follow instructions about eating, drinking, and which medicines to take on the day.

After the operation, the team monitors strength, sensation, walking, wound healing, and pain control. Activity restrictions vary with the procedure, particularly if fusion is added. Use any collar as directed, keep the wound clean, and avoid driving or lifting until the surgical team says these activities are appropriate.

Get urgent help for new arm or leg weakness, rapidly worsening numbness, difficulty breathing, loss of bladder or bowel control, or inability to stand or walk. These symptoms can indicate pressure on neural structures or another serious complication.

Contact the surgical team promptly for fever, increasing wound redness, drainage, a wound that opens, escalating neck pain, severe headache that changes with position, or swallowing that becomes harder rather than easier. Follow the discharge contact route provided by your hospital.

Ask what is compressing the cord, why C1 decompression is proposed, and whether fixation or fusion is anticipated. Clarify which symptoms surgery is intended to improve, how existing cord changes affect expectations, what neck movement may change, and which postoperative restrictions apply to your planned operation.
