What can cause buttock and groin pain?
The pain pattern alone cannot confirm a diagnosis. Hip impingement and a hip labral tear may present with groin pain, clicking, or pain during deep hip bending. Hip osteoarthritis can present with groin or buttock discomfort and stiffness, while back-related nerve irritation may extend through the buttock and farther down the leg.
Clues from the pain pattern
Pain after sprinting, kicking, or a sudden change of direction may point toward a muscle or tendon injury. Deep groin pain provoked by squatting, pivoting, getting into a car, or prolonged sitting can direct attention to the hip joint. Burning pain with tingling or numbness raises a different question about nerve irritation.
A clinician will also consider a groin hernia, urinary symptoms, and pelvic or abdominal causes when the location or accompanying symptoms do not fit a straightforward musculoskeletal pattern.

Temporarily reduce the movement that reliably triggers sharp pain, but keep comfortable daily movement if you can walk safely. Avoid forcing deep stretches or repeatedly testing a painful range. Note whether coughing, stairs, sitting, turning in bed, or sport changes the symptoms, and whether the pain travels below the knee.
Assessment is sensible when symptoms persist, recur, restrict walking, or interfere with sleep. The examination may include the hip, lower back, abdomen, strength, and nerve function; imaging is chosen only when it is likely to clarify the suspected source or guide management.
When to seek urgent care
Seek urgent assessment after an injury if you cannot stand or bear weight, the leg looks deformed, or pain is severe and worsening. Prompt care is also needed for a hot swollen joint, fever, a painful groin bulge that does not settle, or sudden testicular or severe pelvic pain.
Go to emergency care for new loss of bladder or bowel control, numbness around the saddle area, or rapidly developing leg weakness. These neurological symptoms require urgent evaluation regardless of the suspected cause.
