Bullseye Rash: What to Know

A bullseye pattern may contain a central spot, a clearer ring, and an outer edge, but not every circular rash has the same cause. Contact dermatitis can follow exposure to an irritant or allergen; ringworm often forms an enlarging scaly edge; psoriasis, atopic dermatitis, heat rash, intertrigo, and scabies can also create ring-like or target-like areas in particular settings. A new medicine, insect bite, or infection may change the urgency. A clinician considers location, itch or pain, scale, blisters, fever, and how quickly the mark appeared.

Take a clear photograph in good light and note when the rash began, whether it is spreading, recent medicines, travel, outdoor exposure, new products, and contact with animals or affected people. Avoid scratching and pause newly introduced fragranced products. Do not use leftover steroid or antifungal creams until the likely cause is clearer, because treatment can alter the appearance or worsen some infections. Arrange an appointment if the rash persists, expands, recurs, or has scale, pain, or blisters.

Seek urgent assessment for a rapidly spreading target-like rash, fever, significant pain, skin peeling, mouth or eye sores, facial swelling, or breathing difficulty. Get prompt care after a new medicine if the rash is widespread or involves mucous membranes. A rash after a tick exposure, especially with fever or flu-like symptoms, also warrants timely medical advice.

Ask whether the pattern suggests infection, allergy, inflammation, or a medicine reaction; whether a skin scraping or other test is needed; and which changes should prompt urgent review.
