What bulbar polio means

The term bulbar refers to nerve pathways in the lower brain region that coordinate muscles used for swallowing, voice and airway protection. Infection can cause weakness in these muscles, with trouble swallowing liquids or saliva, a weak or nasal voice, slurred speech, reduced cough strength or choking. Other polio features can include fever, headache, muscle pain, stiffness and weakness in a limb. A person may need help protecting the airway and maintaining breathing.

Poliovirus spreads mainly through contaminated hands, food or water and can pass between people even when illness is not obvious. Vaccination is the key way to prevent infection and community spread.

What to do now

If bulbar polio is suspected, seek emergency medical care rather than trying to manage swallowing difficulty at home. Keep the person upright, stay with them and avoid giving food, drink or tablets if they are choking, drowsy or unable to swallow safely. Tell clinicians about recent travel, possible exposure, vaccination history and when symptoms began. Do not share utensils or towels, and wash hands carefully after toileting while advice is arranged.

Clinicians may assess swallowing, breathing, muscle strength and dehydration. Treatment is supportive: the team may provide airway support, fluids, nutrition planning and rehabilitation while monitoring for changes.

When to seek care

Call emergency services immediately for choking, blue or grey lips, pauses in breathing, severe breathlessness, new confusion, inability to swallow saliva or rapidly worsening weakness. A new facial, throat or limb weakness with fever also needs urgent assessment. Do not wait for symptoms to settle, because airway weakness can progress and may be difficult to recognise early.
