Bruising Underneath the Nail

The nail plate is hard, but the tissue underneath contains small blood vessels that can bleed after pressure or impact. This creates a dark patch and may produce a tight, pulsing ache. A bruise can affect a fingernail or toenail and may be called a subungual haematoma. As the nail grows, the patch should travel toward the free edge. A nail can become loose when the impact was strong, and the nail bed may be injured even if the surface is intact.

Keep the nail clean, dry, and protected from friction. A cool pack wrapped in cloth can help with early swelling, and elevation may reduce throbbing. Choose roomy footwear if a toe is affected. Do not make a hole in the nail or pull away a loose section, because this can introduce infection or worsen a nail-bed injury. If pain is more than mild, or the nail is lifting, a healthcare professional can determine whether treatment or an X-ray is appropriate.

Get prompt care for severe or escalating pain, a split or lifted nail, an open wound, a crooked digit, inability to bend or straighten it, or numbness. Seek medical attention if redness, heat, swelling, drainage, or fever develops. A bruise that does not move outward with nail growth, keeps changing, or appears without trauma should be assessed rather than labelled a simple bruise.

Ask whether the nail bed or bone is affected, whether pressure should be released, how to protect a loose nail, and what healing changes are expected. Ask when you should return if pain, colour, or swelling does not improve.
