What should you know about bruised toenail coming off?

A direct blow or repeated pressure can cause bleeding beneath the nail and damage the connection between the nail plate and nail bed. As the nail grows, the detached portion may lift, catch on socks, or fall away. The exposed bed can be sensitive. A new nail may gradually grow from the base, although its appearance can be uneven while the injury resolves. A dark streak or colour change without a clear injury should be assessed rather than attributed to a bruise; see Discoloured Nails.

Do not pull off a nail that is still attached. Wash gently, pat dry, and cover the area with a non-stick dressing if it rubs against footwear. Wear roomy shoes and replace the dressing when damp or dirty. A clinician can trim a loose edge safely and check for nail-bed damage. Do not use household glue or sharp tools. Arrange prompt assessment if the nail is mostly detached, bleeding continues, pain is strong, or the toe cannot move normally. Seek care for spreading redness, warmth, swelling, pus, fever, a foul smell, or red streaking. An urgent review is appropriate after a crushing injury, visible deformity, numbness, or difficulty bearing weight. Diabetes or reduced circulation lowers the threshold for assessment.
