The short answer

Because the tibia lies close to the skin, even a modest impact can feel very painful. The ability to walk does not by itself rule out a crack, so severe focal pain, worsening symptoms, or difficulty bearing weight merits assessment.

Why the shin is so tender

There is little padding over the front edge of the tibia. An impact can injure skin, small blood vessels, and the sensitive tissue covering the bone. A soft swelling may be a collection of blood in the tissue. Pain caused by a single knock differs from shin discomfort that develops gradually with repeated running or jumping; a stress fracture often presents with increasingly localised pain during loading and needs clinical evaluation.

Caring for the injury

Stop the activity that caused the injury and protect the shin from another impact. Use a cold pack wrapped in fabric for short intervals and elevate the lower leg to ease swelling. If the skin is scraped, rinse it with clean running water, cover it with a clean dressing, and watch for spreading redness or discharge. Walk only as comfortably as you can without a pronounced limp. Avoid pressing repeatedly on the sore point to test it, and delay impact exercise until daily walking and gentle ankle movement no longer aggravate symptoms.

When the shin needs medical attention

Seek urgent care if the leg appears bent, a wound is deep or contaminated, bone is visible, or you cannot bear weight after the impact. Rapidly increasing tight swelling, pain that seems out of proportion, numbness, weakness, or a pale or cold foot also needs emergency assessment. Arrange a clinician visit for pinpoint bone pain that persists, pain that worsens with repeated loading, a growing lump, signs of wound infection, or symptoms that are not clearly improving.

What to ask during an examination

Ask whether the location and pattern of tenderness suggest a soft-tissue bruise or a bone injury, and whether an X-ray or other imaging would alter management. Discuss how to protect any wound, how much walking is appropriate, and what functional signs should be present before returning to running or contact sport.

Yes. Some bone injuries do not completely prevent walking, which is why focal tenderness and the injury mechanism matter alongside weight-bearing ability.
