Brown spotting and missed miscarriage

In a missed miscarriage, a pregnancy has stopped developing but the pregnancy tissue remains in the uterus. Some people notice fading pregnancy symptoms or light discharge, while others learn about it during an ultrasound. Brown blood simply indicates blood that has taken longer to leave the body.

Spotting cannot distinguish a missed miscarriage from other causes of pregnancy bleeding. Diagnosis relies on clinical assessment and ultrasound findings, sometimes with repeat imaging or blood tests when pregnancy dates are uncertain or the first result is not conclusive.

Changes in nausea, breast tenderness, or other pregnancy symptoms also cannot provide a definite answer because these sensations naturally vary. Seek an assessment rather than trying to interpret one symptom in isolation.

Contact your maternity service or clinician and describe the amount, duration, pain, clots, tissue, and any dizziness or fever. If a missed miscarriage is confirmed, the team can explain waiting for tissue to pass naturally, using medicine, or having a procedure. The appropriate choice depends on your symptoms, examination, preferences, and whether there are signs of heavy bleeding or infection.

Go to emergency care for heavy bleeding, severe or worsening abdominal or pelvic pain, fainting, pronounced dizziness, weakness, fever, foul-smelling discharge, or feeling very unwell. These symptoms require prompt attention whether or not a miscarriage has been confirmed. If you have only light spotting and feel well, contact your maternity team for guidance and assessment.

Ask how the diagnosis was established, whether another scan is needed, and what to expect with each management option. Clarify pain relief, follow-up, signs of infection or excessive bleeding, and whom to contact outside normal hours. You can also ask about emotional support and when it may be medically appropriate to try for another pregnancy.
