Brown breakthrough bleeding while taking the pill

It does not automatically mean the pill has failed. How consistently you took it, vomiting or diarrhoea, interacting medicines, pregnancy risk and other symptoms determine what to do.

Hormonal contraception keeps the womb lining thin. Small amounts can shed at unexpected times and may turn brown before leaving the vagina. Breakthrough bleeding can occur after starting or changing a pill, taking tablets late, missing tablets, or using packs differently from the prescribed schedule.

Bleeding may instead relate to pregnancy, irritation, a sexually transmitted infection or a growth such as a polyp. The pattern deserves assessment when it persists, returns after previously stable pill use, follows sex, or comes with pain or abnormal discharge.

Continue taking your pill according to its instructions unless a clinician tells you otherwise. Check the leaflet for advice about late or missed tablets, vomiting, diarrhoea and any medicine or herbal product you use. Depending on the pill and what happened, you may need backup contraception or emergency contraception.

Take a pregnancy test when the packet guidance or a pharmacist recommends it, particularly after missed pills or sex without effective contraceptive cover. Keep a note of bleeding days, flow, missed doses and related symptoms. A pharmacist or sexual health clinician can give pill-specific advice because missed-dose rules vary between products.

Arrange a clinical review if bleeding is ongoing, repeatedly occurs after sex, becomes heavier, or is accompanied by pelvic pain, pain during sex, unusual discharge, bad odour or fever. Testing may include a pregnancy test, infection swabs and an examination based on your symptoms.

Get urgent help for very heavy bleeding, fainting, severe abdominal pain, chest pain, sudden breathlessness, coughing blood, or a painful swollen leg. If a pregnancy test is positive and you have bleeding with one-sided pain, shoulder-tip pain or dizziness, seek emergency assessment.

Bring the pill packet and ask whether your dosing pattern still provides contraceptive cover, whether another medicine could interfere, and whether a different pill or method may suit you. Clarify when to test for pregnancy and whether infection testing or a pelvic examination is appropriate.
