Brown Bleeding in the First Trimester: What to Know

Brown bleeding in the first trimester often represents older blood leaving the vagina, but any bleeding during pregnancy deserves attention to its amount, timing and accompanying symptoms. Light spotting can occur when the cervix is more sensitive, including after sex or an internal examination. Bleeding can also occur with changes in an early pregnancy, including miscarriage or an ectopic pregnancy, so colour alone cannot identify the cause.

Fresh blood is often red, while blood that has taken longer to leave the body may look brown or rust-coloured. The shade may change as spotting slows. A small amount without pain may come from the cervix, which can bleed more easily during pregnancy, or from minor irritation. Infection can also cause bleeding, especially with unusual discharge, odour, itching or pain when urinating.

Brown spotting does not by itself confirm that a pregnancy is ending, and it does not rule out a problem. A clinician may consider how far along the pregnancy is, whether the bleeding is increasing, and whether there is pelvic or one-sided abdominal pain.

Contact your maternity service, obstetrician or doctor for personalised advice, even if the spotting is light and has stopped. Note when it began, its colour, whether it is only on wiping or reaches a pad, and whether cramps, dizziness, fever or shoulder pain occur. Use a pad rather than a tampon so you can observe the amount, and avoid putting anything in the vagina until a clinician advises you.

Assessment may include questions about your symptoms, an examination, blood tests or an ultrasound. An early scan may not answer every question immediately, particularly when the pregnancy is very early; follow-up testing can help clarify what is happening.

Seek emergency care now for severe or worsening abdominal or pelvic pain, pain mainly on one side, fainting, marked dizziness, weakness, shoulder-tip pain, or heavy bleeding. These symptoms can signal serious internal bleeding or another urgent problem. An ectopic pregnancy can present with bleeding and pain and needs prompt medical assessment.

Seek same-day medical advice if bleeding becomes bright red or heavier, clots appear, cramps persist, or you develop fever or feel unwell. Do not wait for the colour to turn red before getting help if you feel faint or have significant pain.
