The short answer

Pain may begin immediately after a fall, direct blow, or force that bends the thumb backwards. A fracture cannot be confirmed from pain alone because a severe sprain can produce similar symptoms; examination and imaging may be needed.

What the pain may mean

Tenderness over a specific bone, visible change in shape, or pain when the thumb is gently moved raises concern for a fracture. An injury near the thumb joint can also affect ligaments that stabilise pinch and grip.

Pain at the thumb side of the wrist without a clear injury can have other causes, including tendon irritation. The location, trigger, swelling pattern, and ability to move the thumb help a clinician distinguish these problems.

What to do after an injury

Stop gripping, lifting, or testing the thumb repeatedly. Keep it supported in the position found, remove rings before swelling increases, and place a wrapped cold pack nearby for short intervals without pressing on a deformed area.

Arrange a medical assessment, especially when movement is limited or bone tenderness persists. Do not try to straighten the thumb yourself. A clinician may use a thumb-supporting splint or cast after checking alignment, skin, feeling, and circulation.

Seek urgent care for an obviously crooked thumb, an open wound over the injury, exposed bone, uncontrolled bleeding, or pain that is severe despite support.

Immediate assessment is also needed if the thumb becomes pale, blue, cold, numb, or increasingly difficult to move. These signs may reflect impaired circulation, nerve pressure, or a more complex injury.
