Broken hand swelling: what to expect and when to get help

Inflammation and bleeding around injured bone and soft tissue make the hand and fingers puffy. Swelling can also make a splint, cast, ring, or bandage feel tighter than before.

A fracture triggers inflammation as the body starts repair. Bruising, stiffness, tenderness, and difficulty moving the fingers can accompany the swelling. The amount of puffiness alone does not show whether the bones remain aligned, so follow-up examination and imaging may still be needed.

A wrist fracture can sometimes cause swelling farther into the hand. Other causes, including infection or inflammatory joint disease, may be considered when the pattern does not fit the injury or swelling appears without new trauma.

Keep the hand elevated above heart level when resting, using pillows to support the forearm. Follow the fracture team's instructions about ice, finger movement, pain medicine, and whether the injured hand may be used. Keep a cast or splint dry and do not push objects inside it.

Remove rings from the injured hand early if they come off easily. Do not force a trapped ring; seek clinical help because increasing swelling can restrict circulation. Attend arranged fracture review so fit, skin condition, sensation, and bone position can be checked.

Go to urgent care if pain or swelling is rapidly increasing, the cast feels very tight, fingers become blue, pale, cold, numb, or unusually weak, or you cannot move them as previously instructed. These changes can signal impaired circulation, nerve pressure, or excessive pressure within swollen tissues.

Arrange assessment for fever, spreading redness, drainage, a bad smell from beneath a cast, broken skin near the fracture, or a splint that has become loose or damaged. A newly injured, visibly deformed hand also requires assessment rather than home treatment alone.

Ask whether the fracture position is stable, which fingers and joints should move, when the hand may bear weight, and how to recognise a poorly fitting splint. Also confirm when repeat imaging or hand therapy is planned and which pain medicines are suitable for you.
