What the swelling may mean
An ankle fracture often presents with swelling, pain, bruising, and difficulty bearing weight. Those features can also occur with a severe sprain, so appearance alone cannot confirm which structure is injured. Examination and imaging may be needed to identify the fracture pattern and check alignment.
Swelling may extend into the foot because fluid moves through the tissues, especially when the leg hangs down. A cast, splint, or boot should feel supportive without causing increasing pressure, colour change, burning, or numbness in the toes.
How to protect the ankle
If a break is suspected and has not been assessed, keep the ankle still, avoid putting weight through it, and arrange prompt medical care. Do not try to straighten a visibly deformed ankle.
During recovery, follow the fracture clinic's instructions about the boot or cast, walking aids, weight bearing, and movement. Elevating the supported leg can help fluid drain. Gently moving the toes and knee may reduce stiffness when your clinician has said this is safe. Keep a cast dry and do not place objects inside it.
When swelling needs urgent attention
Seek urgent assessment if pain or swelling is rapidly worsening, the ankle is at an unusual angle, bone is visible, or there is a deep wound. Toes that become blue, pale, cold, numb, or difficult to move may signal impaired circulation or excessive pressure and need immediate care.
Contact the treating team promptly for fever, chills, spreading redness, warmth, discharge, a bad smell beneath a cast, or a cast that suddenly feels too tight. Call emergency services for new shortness of breath, chest pain, faintness, or coughing blood.
