Breakthrough bleeding while on the pill

Keep taking the pill according to its instructions while you review whether any tablets were late, missed, or not absorbed because of vomiting or diarrhoea. Seek clinical advice when bleeding persists, becomes heavier, follows sex, or occurs with pelvic pain, unusual discharge, or pregnancy symptoms.

Hormones in the pill regulate the uterine lining. During adjustment, the lining may shed unpredictably and cause spotting or a light flow. Inconsistent dosing, absorption problems, and some interacting medicines can disturb hormone levels and may also affect contraceptive protection.

Not every episode comes from the pill. Pregnancy-related bleeding, cervical changes, chlamydia, and endometrial polyps are among the possibilities a clinician may consider. A condition such as chlamydia or an endometrial polyp can present with irregular bleeding, but the symptom does not identify the cause on its own.

Use the leaflet for your exact pill to manage missed tablets, stomach illness, or interacting medicines; instructions differ between pill types. If pregnancy protection may have been reduced, follow the stated advice about backup contraception and contact a pharmacist or clinician promptly for personalised guidance. Do not stop midway through a pack solely because of spotting.

Track the flow, tablet timing, pain, discharge, and bleeding after sex. Take a pregnancy test when pregnancy is possible. Book an appointment if the pattern continues, recurs after previously settling, or is difficult to live with; a clinician can assess other causes and discuss a different pill or method if appropriate.

Go for urgent assessment if bleeding is heavy or accompanied by fainting, pronounced dizziness, severe abdominal or pelvic pain, shoulder-tip pain, fever, or a positive pregnancy test. New chest pain, breathlessness, coughing blood, or one-sided leg swelling also requires emergency help because these may signal a blood clot rather than explain vaginal bleeding.

Tell the clinician the pill name, when you started it, dose timing, missed tablets, stomach illness, and all other medicines. Ask whether your bleeding pattern is compatible with adjustment, whether examination or testing is needed, and which alternative formulations or methods fit your medical history if the bleeding remains bothersome.
