Breakthrough bleeding symptoms and warning signs

It may leave a small stain, require menstrual protection, occur after sex, or resemble part of a period. The term describes timing rather than a single diagnosis.

Useful details include whether the blood is pink, red, or brown; whether it is spotting or a sustained flow; where it falls in the menstrual cycle; and whether it follows sex, a missed contraceptive dose, or a change in hormonal treatment. Colour can reflect how recently blood left the uterus but does not establish the cause.

Associated cramps, pelvic pressure, pain during sex, discharge, urinary discomfort, fever, dizziness, or pregnancy symptoms change what needs consideration. Chlamydia can present with bleeding between periods or after sex, sometimes with discharge or pelvic pain. Endometrial polyps can present with bleeding between periods, unpredictable cycles, or heavier menstrual flow.

Mark each bleeding day on a calendar and describe the amount by the menstrual product needed rather than relying only on words such as light or heavy. Add pain location, bleeding after sex, discharge, contraception use, missed doses, medicines, and the date of the last expected period.

Take a pregnancy test if pregnancy is possible. Arrange an appointment if bleeding keeps returning, happens after sex, begins after menopause, or differs substantially from your established pattern. A clinician may suggest pregnancy testing, infection tests, examination, blood tests, or imaging according to the history.

Call for urgent medical help when bleeding is very heavy or comes with fainting, confusion, severe weakness, shortness of breath, or intense abdominal or pelvic pain. One-sided pain, shoulder-tip pain, collapse, or a positive pregnancy test with bleeding needs immediate assessment for an ectopic pregnancy.

Fever, increasing pelvic tenderness, foul-smelling discharge, or feeling acutely unwell should prompt same-day medical advice because infection may require treatment.

Ask which possible causes fit the timing and associated symptoms, whether cervical or infection screening is due, and what testing is appropriate. Clarify what change in flow or pain should trigger urgent care and when to return if initial tests are reassuring but bleeding continues.
