What breakthrough bleeding on the pill can mean

Light spotting can be a pill-related effect, but the timing, amount, associated symptoms, and chance of pregnancy determine whether you need a test or clinical review.

Combined and progestogen-only pills alter the uterine lining in different ways. Irregular shedding may occur after starting, switching brands, skipping a hormone-free interval, or taking a dose later than your pill permits. Brown spotting is often older blood; colour alone cannot identify the cause.

A new pattern can also occur for reasons unrelated to the pill. Chlamydia may present with bleeding between periods or after sex, pelvic discomfort, or discharge, though many people notice nothing. Endometrial polyps may present with unpredictable bleeding or heavier periods. Pregnancy must be considered when pills were missed or absorption was disrupted.

Use the leaflet for the named pill rather than generic missed-dose advice. Note late or omitted tablets, the day of the pack, recent vomiting or diarrhoea, and medicines or supplements started around the same time. A pharmacist can tell you whether extra contraceptive precautions are needed.

Keep a brief bleeding diary that distinguishes spotting from a period-like flow and records cramps, discharge, and sex-related bleeding. If pregnancy is possible, use a home pregnancy test at an appropriate time and repeat or obtain clinical testing if the result does not fit your symptoms. Persistent or recurring bleeding warrants a non-urgent appointment.

Get urgent help if bleeding is rapidly soaking menstrual products, you feel faint or breathless, or you develop intense pelvic pain. Pain focused on one side, shoulder-tip pain, collapse, or bleeding with a positive pregnancy test requires prompt assessment for a pregnancy complication.

Contact a clinician promptly if bleeding accompanies fever, unusual discharge, pain during sex, or a possible sexually transmitted infection exposure.
