Breakthrough bleeding while taking an oral contraceptive pill

It does not by itself show that contraception has failed. Missed tablets, vomiting, diarrhoea, medicines that interfere with the pill, pregnancy, infection, and changes within the uterus can also matter, so the surrounding circumstances guide the next step.

The hormones in an OCP keep the uterine lining thin. That lining may shed unevenly, particularly after starting a pill, changing formulation, taking tablets late, or using packs continuously. The blood may appear pink, red, or brown and can range from marks on underwear to a flow resembling a light period.

Bleeding after sex, pelvic pain, unusual discharge, or bleeding that begins after a previously stable pattern deserves assessment for another cause. Chlamydia can present with bleeding between periods or after sex, although it can also cause no symptoms. Endometrial polyps can present with irregular bleeding.

Continue the pill according to its packet instructions unless a clinician tells you to stop. Check whether any tablets were late or missed and follow the missed-pill directions for that exact product; the advice differs by pill type and by where you are in the pack. Ask a pharmacist about backup contraception if vomiting, diarrhoea, or another medicine may have reduced absorption or effectiveness.

Record the bleeding days, heaviness, pain, missed doses, and any bleeding after sex. Take a pregnancy test when pregnancy is possible, especially after missed tablets. Arrange a routine review if the pattern persists, recurs after settling, or is troublesome.

Seek urgent medical care for very heavy bleeding, fainting, marked weakness, severe or one-sided lower abdominal pain, shoulder-tip pain, or a positive pregnancy test with pain or bleeding. These features need prompt assessment rather than waiting for the bleeding to settle.
