Breakthrough bleeding for two weeks: next steps

Unscheduled bleeding can occur after starting or changing hormonal contraception, when doses are late or missed, or when another medicine affects the method. A prolonged episode also merits checking for pregnancy, infection, cervical changes, and causes within the uterus when clinically appropriate.

Duration does not identify a diagnosis, but it makes the overall impact and pattern important. Endometrial polyps can present with irregular bleeding or bleeding between periods. Chlamydia can present with bleeding between periods or after sex, sometimes alongside discharge or pelvic pain, though symptoms may be absent.

Bleeding may also contribute to tiredness, weakness, breathlessness, or palpitations if blood loss is substantial. Those symptoms matter even when each individual day of bleeding seems manageable.

Book an appointment and bring the name of your contraceptive, when it was started, any late or missed doses, other medicines or supplements, and a record of the bleeding. Include pad changes, clots, bleeding after sex, pelvic pain, discharge, and your usual menstrual pattern.

If pregnancy is possible, take a pregnancy test according to its instructions and share the result. Keep taking prescribed contraception as directed until you receive method-specific advice, unless urgent care instructs otherwise.

Seek emergency help for very heavy bleeding, fainting, severe pelvic or abdominal pain, shoulder-tip pain, marked dizziness, confusion, or breathing difficulty. If pregnancy is possible, bleeding with one-sided pain or faintness needs urgent assessment.

Request prompt medical advice for fever, offensive discharge, worsening pain, increasing flow, or pronounced fatigue and weakness. A routine appointment is appropriate when light bleeding remains stable but has continued for two weeks without red-flag symptoms.

Ask whether the bleeding pattern fits your contraceptive method, whether any medicine could interfere with it, and whether pregnancy testing, infection testing, examination, or blood tests are suitable. Confirm how long to monitor and which new symptoms should change the plan.
