First-trimester bleeding: meaning and warning signs

Although people sometimes call it breakthrough bleeding, that term more often refers to unscheduled bleeding on hormonal contraception. Bleeding in the first trimester can come from the cervix or vagina, occur alongside changes within the uterus, or accompany pregnancy complications.

A clinician considers gestational timing, whether the pregnancy has been seen inside the uterus, the amount and pattern of blood, and symptoms such as cramps, one-sided pain, dizziness, fever, or discharge. Cervical irritation after sex may cause light bleeding. Cervical infection is another possibility; chlamydia can present with bleeding after sex or unusual discharge and may also be symptomless.

Neither spotting nor a heavier flow confirms an outcome on its own. Depending on the circumstances, assessment may involve an examination, ultrasound, pregnancy-hormone blood tests, or planned follow-up.

Call your maternity team or healthcare provider and describe whether the blood is only seen when wiping, marks a pad, or is increasing. Report clots or tissue, the location and strength of pain, dizziness, shoulder-tip pain, fever, recent sex, and any previous pregnancy complications.

Keep using pads to observe the flow and have your medication list available. If you do not yet have maternity care arranged, contact an urgent medical service for guidance on the appropriate assessment location.

Go for emergency care if bleeding is heavy, pain is severe or mainly on one side, or you develop shoulder-tip pain, fainting, marked dizziness, weakness, confusion, or breathing difficulty. Sudden deterioration needs an emergency response even if earlier spotting seemed minor.

Seek prompt clinical advice for bleeding with fever, chills, offensive discharge, worsening cramps, or persistent symptoms. The urgency comes from the bleeding pattern and accompanying symptoms, not from whether a particular cause is common.

Ask what the assessment has established about the pregnancy's location and development, whether repeat blood tests or ultrasound are planned, and who will review the results. Confirm which changes mean you should return immediately and what to do if bleeding continues outside clinic hours.
