Breakthrough bleeding and pregnancy

The term breakthrough bleeding usually describes unscheduled bleeding while using hormonal contraception. Once pregnancy is confirmed, clinicians describe the symptom by its timing, amount, and associated features. Light spotting can have several explanations, yet bleeding alone cannot show whether a pregnancy is progressing normally.

Early-pregnancy bleeding may come from the cervix, which can bleed more easily, including after sex. It can also accompany pregnancy loss or a pregnancy developing outside the uterus. Later bleeding has a different range of possible causes and needs maternity assessment. Infection may also cause spotting: chlamydia can present with bleeding after sex or between periods, although symptoms cannot establish the diagnosis.

Bleeding while taking contraception does not prove pregnancy and does not rule it out. If pills were missed, taken late, affected by vomiting or diarrhoea, or used alongside an interacting medicine, follow the product instructions and test at the appropriate time.

If pregnancy is possible, take a home test. Contact a healthcare professional if bleeding continues or if the test is positive. If pregnancy is already confirmed, tell your maternity team about the colour and amount of blood, clots or tissue, pain location, dizziness, and your pregnancy stage. Use a pad rather than an internal menstrual product so the flow is easier to observe. Do not stop prescribed medicine without advice.

Seek urgent medical care for heavy bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, pronounced dizziness, fever, or feeling acutely unwell. These symptoms require prompt assessment whether a pregnancy test is positive, negative, or not yet taken. In a confirmed pregnancy, follow the urgent-contact instructions provided by your maternity service for any bleeding.

Ask whether you need examination, blood tests, infection testing, or an ultrasound, and when each test can provide useful information. Confirm whom to contact if bleeding increases or pain appears. If you were using contraception when pregnancy occurred, bring the product name and describe exactly how you used it.
