Breaking water as part of induction

Short answer

In a breaking-water induction, a clinician deliberately opens the amniotic membranes to encourage labour, after checking that the cervix and baby's position make the procedure suitable.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Other name
Artificial rupture of membranes or amniotomy.
Afterwards
Fluid continues to drain and labour response is monitored.

The main point

The procedure is also called amniotomy or artificial rupture of membranes. It may be one part of an induction plan rather than the only step, and it requires a setting where the baby's heartbeat and labour response can be assessed.

Where it fits in an induction plan

If the cervix is closed or not ready, another method may be offered first to soften and open it. When the cervix allows access and the baby's presenting part is in an appropriate position, a trained clinician can make a small opening in the sac during a vaginal examination.

Fluid then drains, and contractions may begin or strengthen. Some people need contraction-stimulating medicine afterwards. The proposed sequence depends on examination findings, your health, the baby's wellbeing, and the reason birth is being recommended.

Before and after the membranes are opened

Before consenting, make sure you understand the purpose, what the examination involves, available alternatives, and the likely next step if contractions do not establish. Mention previous caesarean or uterine surgery, medicines, allergies, bleeding, and any recent reduction in fetal movements.

After amniotomy, expect ongoing fluid leakage and use the pads provided. Tell staff if you need help changing position or managing discomfort. Follow guidance on movement, food, drink, examinations, and monitoring because these details vary with the induction method and your clinical needs.

When to alert staff without delay

Report heavy bleeding, severe pain that persists between contractions, fever or chills, shortness of breath, faintness, a marked reduction in fetal movement, or pressure with an urge to push. A sudden sensation of something in the vagina also needs an immediate check.

Before your scheduled induction, phone maternity care immediately if your waters open spontaneously, particularly before term or with unusual-coloured fluid. Preterm labour can include ruptured membranes, tightening, pelvic pressure, and low back discomfort.

Questions for planning the induction

Ask which finding makes amniotomy appropriate, how the baby's heartbeat will be observed, and whether you can remain mobile. It is also reasonable to ask how long the team will wait for contractions, when medication may be suggested, what pain relief is available, and which events could change the delivery plan.

Questions people ask

Is breaking the water the same as a membrane sweep?

No. A membrane sweep separates membranes near the cervix without deliberately opening the amniotic sac, while amniotomy creates an opening so fluid drains.

What if my cervix is not ready for amniotomy?

The maternity team may discuss a cervical-ripening method first or another plan suited to your findings. Ask what each option involves and what reassessment will guide the next step.
