What should you know about brain syphilis?

Syphilis is an infection that can spread beyond the initial skin or genital symptoms. When it affects the brain, spinal cord, eyes, or nerves, doctors may call it neurosyphilis. Possible features include headache, altered thinking, personality change, balance difficulty, weakness, numbness, vision problems, hearing changes, or seizures. These signs can have many causes, so they do not identify syphilis on their own.

Assessment may combine an examination of the nervous system with blood tests and, in selected cases, a sample of fluid around the brain and spinal cord. STI screening can help check for syphilis and other infections. The clinician also considers past treatment and the timing of symptoms.

Arrange prompt medical review if syphilis is known or possible and there are neurological, eye, or hearing symptoms. Share any previous antibiotic treatment, test results, allergies, pregnancy, and details of possible exposure. Do not stop or change antibiotics without medical advice.

Treatment uses antibiotics selected for nervous-system infection, often given in hospital or through a monitored route. The team may involve infectious-disease, neurology, eye, or sexual-health clinicians. Follow-up checks help confirm that the response is satisfactory and identify symptoms that need further care.

Go to emergency care for sudden weakness, trouble speaking, a new seizure, severe confusion, fainting, sudden vision loss, or a severe headache with neck stiffness. New eye pain or hearing loss also warrants urgent assessment because untreated infection can threaten these functions.
