Recognising bowed-out alignment

Clinicians call this alignment genu varum. Some people have an even curve without pain or limitation, while others develop altered loading through the knees, ankles or feet.

Why the pattern varies

Leg alignment reflects the shape and rotation of the thighbones and shinbones as well as hip, knee, ankle and foot position. The apparent gap at the knees is therefore only one part of the examination. Walking observation may reveal a foot that turns inward, a side-to-side thrust at the knee or uneven loading.

In early childhood, a symmetrical bow may be a developmental stage that evolves with growth. Concern rises when the curve progresses, differs substantially between sides, appears outside the expected growth pattern or occurs with pain and functional change. In an adult, a new bow may accompany joint wear, previous injury or altered bone alignment.

Managing comfort and function

Choose activity according to comfort and avoid repeatedly pushing through pain. Supportive footwear may improve stability, while a record of pain location, walking tolerance and visible changes can make an appointment more useful. Do not buy a corrective brace solely from photographs or general advice.

Treatment targets the reason for the alignment and its effect on daily life. Rehabilitation may build hip and leg strength, refine balance and improve movement control. It does not remodel a fixed bony curve. When deformity is substantial and creates joint or functional problems, an orthopaedic surgeon may discuss realignment options after detailed evaluation.

Changes that merit assessment

Schedule a review for increasing curvature, a clear left-to-right difference, ongoing knee or ankle pain, limping, repeated falls, stiffness or reduced participation in usual activities. In a growing child, bring growth and developmental history to the visit if available.

Urgent care is needed when severe pain or deformity follows an injury, the person cannot stand on the leg, or the limb swells rapidly. Fever with a red or hot joint, sudden weakness or numbness, and a cold, pale or blue foot also require immediate evaluation.

Ask whether the alignment is expected for the person's age, which bone or joint contributes most, and whether it is affecting joint loading. Clarify the purpose of any imaging, exercise programme, brace or referral before starting treatment.
