The key difference

With bow legs, the knees remain apart when the ankles are together. With knock knees, the knees meet while the ankles remain apart. Both patterns can appear during childhood development, but timing and clinical context matter.

How clinicians interpret the pattern

Leg alignment changes as children grow, often moving through bowed and then knock-kneed stages before settling. A clinician considers the child's developmental stage rather than applying an adult standard. In adults, alignment may reflect long-standing anatomy, previous injury or joint change.

Examination compares the sides, observes the kneecaps and feet, and checks hip, knee and ankle movement. Walking may reveal rotation, limping or altered balance that is not obvious while standing. The terms describe shape; they do not identify the underlying cause.

What to observe and do

Look for symmetry, comfort and change over time rather than repeatedly measuring at home. Note limping, tripping, reduced play, difficulty running or pain around the hips, knees, ankles or feet. Let a child stand naturally; pushing the knees or ankles together can give a misleading impression.

If assessment finds a movement-control issue, Paediatric Physiotherapy may address strength, balance and activity skills. Exercises do not reshape a fixed bone deformity. Significant structural malalignment may be monitored by an orthopaedic specialist, with Osteotomy considered only in selected circumstances.

Reasons to get checked

Arrange assessment if the alignment is much greater on one side, becomes more pronounced, persists outside the expected growth pattern, or occurs with pain, instability or activity limitation. New alignment change in an adult also warrants evaluation.

Urgent care is needed after trauma when the leg looks newly deformed or cannot take weight. Fever with a hot swollen joint, severe unexplained pain, new numbness or a cold discoloured foot also requires prompt medical attention.

What to clarify at the visit

Ask whether the alignment is expected for the person's age, whether it comes from bone shape or rotation, and if both limbs are developing similarly. Find out whether observation is sufficient and which symptom or visible change should lead to reassessment.
