Therapy for Bow Legs

Short answer
Bow-leg therapy can improve strength, movement and comfort, but exercises do not reliably reshape a fixed bony alignment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Therapy targets
- Function, capacity, mobility, balance and symptom control
- Structural limit
- Exercise cannot consistently remodel fixed bone alignment
- Plan basis
- Cause, age, symptoms and activity goals
What therapy can and cannot do
A useful plan targets the person's actual difficulties, such as reduced hip control, limited ankle movement, poor balance, pain with loading or an inefficient walking pattern. Treatment should follow assessment rather than trying to force the knees into a straighter position.
Why the cause guides treatment
Genu varum describes outward knee alignment, not a single disease. In children, the pattern may relate to development or a growth-related problem. In adults, longstanding bone shape, previous injury, joint wear or another condition may contribute. Each situation has different management priorities.
Physiotherapy works mainly on modifiable features around the alignment. It can build muscle capacity, improve joint mobility where restriction exists, retrain movement strategies and pace activity. A brace or orthotic may help selected people manage loading, but it should be fitted for a defined clinical aim.
Starting a sensible therapy plan
Begin with an examination of standing posture, walking, joint range, strength, balance and the activities that trigger symptoms. The therapist can then choose exercises and adjust their difficulty according to response. Sharp joint pain, increasing swelling or a worsening limp are reasons to stop and reassess rather than push through.
For a child, therapy can support play, coordination and movement skills when these are affected, while a paediatric or orthopaedic clinician evaluates the underlying alignment. For an adult with a fixed deformity and persistent limitations, rehabilitation may be used alone or around a specialist discussion about realignment surgery.
When further assessment is needed
Arrange medical review for progressive or clearly uneven bowing, pain that continues despite an appropriate programme, recurrent knee instability, or declining ability to walk and manage daily tasks. Children should be reviewed when alignment changes unexpectedly, movement development is affected or growth appears uneven.
Seek urgent care after a significant injury, for sudden inability to bear weight, or when a joint is hot and swollen. Fever with limb pain, new numbness, marked weakness, or a pale or cold foot also needs prompt medical attention.
Questions to shape treatment
Ask which findings are changeable, how each exercise connects to a functional goal, and what response indicates that the dose is suitable. Also clarify how progress will be measured, whether another clinical assessment is needed, and which symptoms should pause the programme.

Questions people ask
What should you know about bow leg therapy?
Therapy is most useful when it addresses specific movement, strength or comfort problems. The underlying cause determines whether observation, rehabilitation, medical treatment or surgery is relevant.
Which exercises are used for bow legs?
Exercise selection may address hip and leg strength, ankle mobility, balance or gait. There is no universal routine because the source of alignment and the person's functional needs differ.
Can a brace correct bow legs?
A brace may have a role in selected circumstances, particularly when prescribed for a specific diagnosis. It should not be bought solely from appearance without clinical assessment and fitting.
