Understanding the Bow-Leg Position

How the structures involved in Bow Legs differ from normal

Short answer

A bow-leg position means the knees stay apart when the legs are straight and the ankles are brought near each other.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Genu varum

What the position looks like

Clinicians call this alignment genu varum. It is assessed in a relaxed, natural stance because deliberately rotating the hips, turning the feet or forcing the ankles together can exaggerate or conceal the apparent curve.

Why the whole limb matters

The visible gap between the knees is only one part of alignment. Hip rotation, the shape of the femur and tibia, knee-joint orientation, ankle position and the direction of the feet can all affect how the legs look from the front.

Static posture does not show how the limb manages load. During walking, an examiner observes the direction of the kneecaps and feet, side-to-side movement at the knee, stride pattern and balance. This separates a flexible posture from a more fixed bony alignment and identifies functional strain.

How to observe and respond

Stand comfortably on a level surface with weight shared between both feet, then notice whether the bowing is similar on each side. Do not repeatedly press the knees inward or use improvised straps, braces or shoe wedges. Those measures may place stress elsewhere and cannot address every cause.

If the position is associated with discomfort or movement difficulty, note which activities bring it on and where symptoms occur. A physiotherapist or orthopaedic clinician can examine joint mobility, muscle capacity, balance and gait before suggesting an appropriate plan.

When the position needs review

Seek a routine assessment if the curve becomes more noticeable, differs clearly between the legs, or accompanies recurring knee, hip, ankle or foot symptoms. A limp, knee instability, reduced walking tolerance or difficulty with everyday movement provides further reason to be examined.

Get prompt medical help for sudden deformity after trauma, inability to bear weight, severe swelling, a visibly displaced joint, or new numbness or colour change below an injury. Do not attempt to straighten an injured limb yourself.

Useful questions at an appointment

Ask whether the alignment arises mainly from bone shape, joint position or rotation, and whether it changes during walking. Other practical questions cover the source of any pain, which activities are suitable, whether rehabilitation could improve function, and how progression will be recognised.

Questions people ask

What should you know about bow leg position?

It describes outward alignment around the knees, but appearance alone cannot locate its source. Symmetry, joint movement, gait and symptoms give the position clinical meaning.

Can foot position make legs look more bowed?

Yes. Turning the feet or rotating at the hips can change the appearance, which is why clinicians examine the whole limb in standing and walking.

Should I force my knees together as a stretch?

No. Forceful self-correction may irritate joints or soft tissues. Use exercises only after a professional identifies the movement or capacity that needs attention.
