Bone scans for osteomyelitis: what patients should know

The test uses a small amount of radioactive tracer that collects where bone turnover or blood flow is increased. Infection can create this pattern, although a recent fracture, surgery, arthritis, or another bone process may look similar.

An area of increased tracer uptake is often called a hot spot. Its location and pattern help the clinician decide whether infection remains a concern, but the image must be interpreted alongside symptoms, examination findings, blood tests, and other imaging.

Previous operations, implants, healing injuries, and poor circulation can affect interpretation. The clinician may request more targeted imaging or a sample from the affected area when identifying the organism would change treatment.

Tell the imaging team if you are pregnant, could be pregnant, are breastfeeding, or recently had a nuclear medicine test. Also mention surgery, fractures, implants, medicines, and the exact site of pain or any wound.

Tracer is given through a vein, and images may be taken at different stages. You may have a waiting period before later pictures. Follow the centre's instructions about fluids and emptying your bladder, then arrange follow-up so the result is considered with the rest of your assessment.

Seek prompt assessment for fever or chills with worsening bone pain, spreading redness, increasing swelling, pus from a wound, or a wound that exposes bone or follows recent surgery. Do not wait for a scheduled scan if you feel rapidly more unwell.

Emergency care is appropriate for confusion, fainting, severe weakness, breathing difficulty, or rapidly worsening illness. These warning signs require assessment because a serious infection can affect more than the painful area.

Ask what the scan pattern showed, which other conditions could explain it, whether more imaging or a tissue sample is needed, and when treatment decisions will be made. If you have an implant or previous bone surgery, ask how that history changes interpretation.
