Blisters in the diaper area: causes, care, and warning signs

Keep the skin clean and dry, reduce rubbing, and arrange timely advice from a healthcare professional. Do not pop blisters or apply a leftover prescription, essential oil, antiseptic, or strongly fragranced product to raw skin.

Ordinary irritant diaper rash usually affects skin exposed to urine and stool and may spare deeper folds. A bright red rash involving folds with small nearby spots can occur with yeast overgrowth. Fragile blisters, honey-coloured crust, pus, sharply worsening redness, or open sores raise concern for another infection or more severe injury.

A clinician may need to see the distribution and intact lesions to distinguish irritation, infection, allergy, friction, and less common blistering disorders. Clear photographs taken before cleaning can be useful if the blisters change quickly.

Change wet or soiled diapers promptly. Rinse with lukewarm water or use an unperfumed gentle cleanser, then pat rather than rub. Allow supervised diaper-free time and fasten the next diaper loosely so air can circulate and edges do not chafe.

A plain barrier ointment can shield unbroken irritated skin from moisture, but avoid coating open blisters unless a clinician recommends it. If an antifungal is advised, use the named product for the suspected cause and do not combine it with a steroid unless specifically directed.

Seek urgent medical care if the baby is difficult to wake, feeding poorly, breathing unusually, producing much less urine, or has fever with a rapidly spreading rash, purple areas, widespread blisters, or peeling skin. Fever or blisters in a newborn require prompt assessment.

Contact a clinician soon for pus, bad odour, increasing warmth or swelling, severe pain during cleaning, bleeding sores, grouped blisters, honey-coloured crusts, or a rash extending well beyond the diaper area.

Explain when the rash began, whether it started in the folds or exposed skin, recent diarrhoea, fever, feeding and urine output, new wipes or diaper brands, medicines, and treatments already tried. Ask whether the appearance suggests yeast or bacteria, whether a swab is needed, and exactly where each recommended cream should go.
